Schedule a video call. Click the schedule button to select a new patient appointment time. You will receive a confirmation email that contains a link for a New Patient Intake Form, consents, and practice policies. You will also get access to your Patient Portal. You will need to fill out the New Patient Forms and complete your profile on our Patient Portal before your first visit.

Our clinic will email you a link for the video call a few minutes before the appointment time. Your login information is the same as your Patient Portal. During the call, we will review your detailed history and discuss all possible treatments. This appointment is all about gathering all the information and determining the best treatment plan to help reach your goals.

Any medication/supplies will be shipped directly to you and is included in the monthly/quarterly price.
You have access to the clinic and provider with questions and concerns while actively getting treatment. Follow up video visits will be scheduled based on your individual needs.
